Form not displaying on mobile

Please somebody help
Form display on website but no where to be found on the mobile set.
I really need help urgently

What is url of your site?
It may be that your form is a module that is published in position of desktop site only.

Yes, your form is a module published in 'left' position. Make copy of this module and publish it in 'mj_all_header' position (for example).

I've selected Standard version in iPhone (just to see how it works) but now I can not return to Mobile Version. This occured in Safari browser.
Can you help me?
